Insurance and payment Information
Dr. Hitchmoth is a contracted provider for many insurance companies. There are hundreds of insurance plans so please call your insurance company directly to verify your coverage. We are happy to answer general insurance questions but we are unable to precertify payment coverage.
FEES: Dr. Hitchmoth provides a wide variety of services and fees vary according to service. We are happy to provide you with a fee estimate, however, we cannot predict the level of service required until your visit occurs.
TIME OF PAYMENT: Unless prior arrangements are made or insurance is being billed, payment for services is expected at time of service.
YOUR INSURANCE COVERAGE AND OUR OFFICE POLICY: Your insurance policy is a relationship between you and your insurance company. Please be aware that some or all of the services we provide may not be considered necessary by your insurance company. Your insurance company will determine which benefits are eligible for payment after your claim has been submitted. Co-payment will be collected from you at the time of service, however, you may be billed additional fees based on your insurance coverage.
Discounts: We offer a variety of discounts for services for Dartmouth employees, students and the uninsured. Please call for more information.
CONTACT LENS FEES: Eye examinations for contact lens wearers requires additional measurements and testing thus the exam fee is higher than an examination for vision care or glasses. When new patients do not know what contact lens brand they are wearing or if any patient requires refitting into a new contact lens brand, the doctors charge a refitting fee based on the complexity of your contact lens refit. Included in the refitting is the time required follow up contact lens evaluations and trial or fitting contact lenses. Feel free to ask the staff for an estimate the costs of your contact lens refitting. Insurance typically does not cover the cost of contact lens fittings.

Dr. Hitchmoth does not accept following "discount" vision plans for medical care, however, The optical at Dr. Sam's does provide contracted coverage for glasses for the following plans:
- Davis Vision
- FEP Blue Vision (Federal Employees)
